ASSESSMENT PROCEDURE

  1. Register your child through the Google form link provided by the school for new students.
  2. A school representative will send you an email within 48 hours asking you to accomplish
    a Student Information Sheet. Submit the Student Information Sheet as filled out, together with the following required documents for assessment, to the Front Desk Officer or through email to office@magisacademy.edu.ph:
    a. copy of the child’s birth certificate
    b. copy of the recent Report Card
    Original copies should later be forwarded to the school and shall form part of the school records upon enrollment. Alternatively, parents may bring their child with them to the school office and may fill out the Student Information Sheet onsite while submitting copies of their child’s birth certificate and latest Report Card to the Front Desk Officer.
  3. The child will be assessed onsite (interview and written assessment) in the school premises by the Guidance Officer or school administrator on a scheduled date. The assessment fee shall be paid on or prior to the scheduled date of the child’s assessment.
  4. If the child passes the assessment, the child may proceed to enroll for the incoming school year.

ENROLLMENT/ADMISSION PROCEDURE

  1. For enrollment, the following documents must be accomplished and submitted to the
    Front Desk Officer:

    a. Registration Form
    b. Latest Report Card (Original copy)
    c. 1 x 1 Photo with white background
    d. Certificate of Good Moral Character or equivalent document
    e. Recommendation Letters (Except for Preschool)
    f. Student Health Record
    g. Questionnaire
    h. Data Privacy form

    For students under the Foreign Student Program (FSP) and Exceptional Students
    Educational Program (Estep), the following additional documents should be submitted:

    a. Photocopy of O.T/ Speech Report/s (Estep)
    b. Photocopy of Developmental Assessment Report (Estep)
    c. Photocopy of Passport – 2 copies (FSP) (bio-page, admission, authorized stay)
    d. Photocopy of ACR I – Card – 2 copies (FSP)
    e. Original copy of Notarized School Records (FSP)

  2. Once the documents have been submitted, the enrollment fee may be paid to the school cashier or via online bank transfer to confirm the child’s enrollment for the incoming school year.
